Internal Medicine is a specialty that focuses on the health, wellness, and medical condition of the adult individual as a whole. We monitor all of your organs and systems to ensure they are functioning properly. If they are not, we will refer you to the right specialist or specialists for your condition and coordinate care across your entire team of healthcare providers and specialty physicians. With us, you will always have a one point of contact who has a complete understanding of your health and can ensure you are getting the medical attention you need. New Patient Information

We are accepting new patients. If you are looking for a new internal medicine physician / primary care doctor or to improve your health and wellness, we invite you to call our office with questions or to schedule an appointment.

When you call to schedule an appointment, please have your insurance information available. We accept most insurances, including Medicare. If you have questions about insurance, we encourage you to ask us when you call.

In order to help Dr. Schwartz understand your past medical history and any current conditions, please have your medical records sent to our office prior to your first visit. Your medical records can be faxed or mailed to our office.

If you have any questions about establishing care with Dr. Schwartz, please do not hesitate to call us. We look forward to working with you.

Prescription Refills

If you need a prescription refill or a feel you need a referral to a specialist, please call our office to request your refill or referral. We will notify you if we can process your request or if you will need to see Dr. Schwartz first.

Continuity & Coordination of Care

For continuity of your care and coordination amongst your healthcare providers, Dr. Schwartz will oversee your care as your primary care doctor. Should you need to see a specialist, she will refer you to the appropriate provider and communicate and work with that provider to ensure you receive the care you need.

Health Insurance & Payment Information

We accept most health insurances, including Medicare, as well as cash and credit card payments. Please call us to learn more about health insurance plans we accept and payment options.

Appointments & Accessibility

We are accepting new patients. Please call our office to schedule an appointment with Dr. Schwartz. If you feel you need to see Dr. Schwartz the same day, please call us right away so that we may do our best to accomodate you.
